Case Details
This patient in his 40s presented with concern of lower lid bags. He looked tired and sleepy and just did not look like himself.
On evaluation, Dr. Phan noted mild orbital fat herniation and volume loss of the tear trough on both sides, causing uneven contour of the lower eyelids and thus mild shadowing of the tear troughs. This was the culprit for his tired appearance. In addition, Dr. Phan noted excess skin of the upper eyelids, overlapping the eye opening, causing the sleepy appearance. In actuality, the eye openings were nice and big under the eyelid hoods.
To address the upper and lower dermatochalasis and lower lid volume loss, Dr. Phan recommended blepharoplasty to remove excess skin and sub-orbiculares oculi fat elevation and fat transposition to move cheek fat up and orbital fat down, respectively. Moving those fats in those respective directions filled in the tear trough depression and smoothed the lower lid contour.
