Case Details
This patient in her 40s presented after several years of gradually worsening upper eyelid sagging. Her primary concern was that the upper eyelid skin made her appear tired. Non-surgical modalities, including Botox injections above the brows and between the eyes, had not improved the upper eyelid skin folds.
On evaluation, Dr. Phan noted significant anatomical differences between the two upper eyelids. The left eyelid had two natural creases, a slightly longer horizontal axis, and subtle hollowing of the upper sulcus. The right eyelid had thicker skin and a more defined crease. Given these differences, careful surgical planning was necessary to improve symmetry while preserving her natural appearance.
For the upper eyelid excess skin, Dr. Phan recommended individualized upper blepharoplasty. The amount of skin to be removed was carefully measured and marked on each side, taking into account the patient’s asymmetric eyelid anatomy. The goal was to improve the heaviness of the upper eyelids without over-resecting skin or altering the natural contours of her eyes.
In this case, the upper blepharoplasty was performed under local anesthesia. Carefully measured amounts of excess skin were removed from each eyelid with minimal tissue trauma, while special attention was given to maintaining symmetry and preserving the patient’s natural eyelid characteristics.
