Are all blepharoplasties the same?

Short answer is… No.

All blepharoplasties involve skin removal from the upper and/or lower eyelids. However, not all blepharoplasties are the same. The right approach depends on what is causing the heavy, tired, or aged appearance around the eyes.

1. Upper and lower blepharoplasty are different

  • Upper blepharoplasty addresses excess upper eyelid skin, and sometimes fat.
  • Lower blepharoplasty addresses puffiness, loose skin, or under-eye bags.

2. Not every heavy upper eyelid needs the same surgery

  • Some patients mainly have redundant skin.
Some have brow descent pushing tissue into the eyelid.
Some have true eyelid ptosis, where the eyelid margin itself sits too low.
These can look similar to patients but require different treatments.

3. Technique varies by anatomy

  • A patient with thin skin and a high crease may need a different approach than someone with thicker skin, a low crease, or fuller eyelids.
Some patients benefit from crease-based skin excision, while others may be better served with a sub-brow approach or combined brow support.

4. Lower blepharoplasty is not one-size-fits-all either

  • Some patients need fat removal or repositioning.
    Others need skin tightening.
Others mainly have volume loss, pigmentation, or cheek descent, which surgery alone may not fully fix.

5. Cosmetic goals matter

  • One patient wants a sharper crease.
Another wants subtle rejuvenation without looking “done.”
Another wants to preserve ethnic identity and natural eyelid character.
The same word—blepharoplasty—can describe very different surgical plans.

6. The best blepharoplasty is the one matched to the cause

  • Good results come from diagnosing correctly, not just removing skin.
If the brow is low, lifting only the eyelid may not solve the problem.
If the eyelid opening is small from ptosis, skin removal alone may still leave the eyes looking tired.

Conclusion

Blepharoplasty is not a single, standardized operation. It is a tailored procedure designed around each patient’s anatomy, aging pattern, and goals. The most natural results come from choosing the right operation for the right reason.
