Case Details
This patient in her 60s presented with concerns about looking tired and sleepy, as well as bags of her lower eyelids.
On evaluation, Dr. Phan noted severe upper eyelid ptosis, along with herniated orbital fat and tear trough hollowing of the lower eyelids. The upper eyelids drooped because the muscles responsible for lifting the eyelids had thinned, stretched, and weakened over time. The contour of the lower eyelids became uneven, appearing fuller where orbital fat herniated forward from the deeper orbital space and hollowed where other fat descended into the midface. To address these changes, Dr. Phan recommended bilateral internal ptosis repair and lower lid blepharoplasty with fat transposition and horizontal tightening.
