Case Details
This patient in his 70s presented with a smaller left eye, making him appear tired and imbalanced.
On evaluation, Dr. Phan noted drooping of the left upper eyelid and excess skin on the right worse than left upper eyelid. The former was due to thinning, stretching and weakening of the muscles responsible for lifting the eyelid up, while the latter was due to thinning, stretching and redundancy of the skin as well as to the drooping of the eyebrows. The right brow was droopier than the left and pushing down into the eyelid more on this side.
To address the lid ptosis and dermatochalasis, Dr. Phan recommended either left ptosis repair with right blepharoplasty or bilateral brow lift. Ptosis repair would involve shortening and tightening the levator muscle, so it could lift the eyelid more effectively. Blepharoplasty would involve removing excess upper eyelid skin. For this patient, a brow lift would involve placing incisions within the forehead wrinkles and removing a measured amount of skin to allow the brows to be come up.
